The Federal Corporate Transparency Act requires all community associations to report the following information for each of the association's directors and senior officers by December 31, 2024:

Full legal name
Date of birth
Complete residential address
Personal identification number (e.g., Drivers License number and issuing state, Passport number, or another approved ID)
Copy of the ID (i.e., the DL, Passport, or state ID listed above)

I understand the Corporate Transparency Act is a Federal requirement and failure to comply can lead to significant fines to individual Board members and the Association.

Remember, volunteering to be a board member requires 5 to 10 hours of volunteer time per month for a full term. Only submit this form if you can meet this time commitment. Your name will be listed on both the absentee voting ballot (mailed with the Annual Meeting notice), as well as the voting ballot to be handed out at the Annual Meeting.
